US woman renews driver's licence, gets new ID with pic of empty chair

A woman in Tennessee, US, received her renewed driver's licence with a photo of an empty chair on it instead of her face. The error occurred as the empty chair was the last photo mistakenly taken and saved to Jade Dodd's file during her last visit to Motor Vehicles Department. She was later issued a new licence with her photo.
