What is known about creator of Microsoft's 'Blue Screen of Death'?

The identity of the creator of Microsoft's 'Blue Screen of Death' (BSOD) has been debated for long. According to reports, former Microsoft CEO Steve Ballmer was responsible for its creation. Microsoft developer Raymond Chen however claimed to have coded the BSOD into Windows 95. Another report claimed ex-Microsoft developer John Vert created the modern Windows blue screen in version 3.1.
