The high-level committee constituted by Andhra Pradesh government for LG Polymers gas leak in Vizag, which led to 12 deaths, has submitted its report. The panel said that poor design of the tank at the plant, inadequate refrigeration and faulty cooling system, poor safety protocols and total breakdown of the emergency response were root causes of the tragedy.
