Mechanical and design issues contributed to the crash of a Lion Air Boeing 737 MAX jet last October which killed 189 people, Indonesian investigators told the victims' families. They criticised certification procedures for the jet, saying that a flight-control mechanism was approved based on incorrect assumptions. "Deficiencies" in the flight crew's communication was also said to be a factor.
