The Election Commission of India has shared sample forms for the nationwide voter list revision exercise. The voters will have to fill in their details of the last Special Intensive Revision (SIR) exercise conducted in 2004 and will also have to paste their current photo in the form. Voter's Aadhaar number has been made optional in the form.
