To convert physical shares into demat, open a demat account with the same name. Fill a Demat Request Form (DRF), mark the certificates as 'Surrendered for Dematerialisation' and submit all documents to your DP (depository participant). The registrar verifies them, and once approved, shares are credited to your account in 15 to 30 days.
