A 19-year-old was fatally pulled into a meat grinder while cleaning equipment at Tina’s Burritos plant in Vernon, California, on July 13. The machine suddenly activated during his after-hours shift. Multiple agencies, including Cal/OSHA, are investigating possible safety failures and whether proper lockout/tagout procedures were followed. The victim’s name has not yet been released.
