The safety driver of a self-driving Uber car that hit a woman in Arizona earlier this year and led to her death, was watching a TV show on her phone at the time of crash, police said. The collision that killed the 49-year-old woman was "entirely avoidable", the police added. The driver looked up half a second before the crash.
