To ensure the family can access a person's investments/assets after their death, they can maintain a consolidated record of bank accounts, FDs, mutual funds, shares, bonds, insurance policies and other investments. The record can include details of institutions and platforms, account/folio numbers and nominee details. It should be kept updated and securely accessible to a trusted family member(s).
