What is the cause of back pain during a fever?

Fever-induced back pain is often musculoskeletal or viral in origin. However, differential diagnoses include pyelonephritis, epidural abscess, and meningitis. Clinical red flags such as hematuria, neurological signs, or persistent pain require urgent evaluation. Most non-specific pain resolves with hydration, rest, and antipyretics.
