Why did US swimmer Alex Walsh lose Olympics bronze moments after win?

US swimmer Alex Walsh was disqualified and lost a bronze medal moments after finishing third in the women's 200-meter individual medley at the Paris Olympics after a review cancelled her result. She was disqualified for an inappropriate turn from backstroke to breaststroke. She did not complete the backstroke leg of her race on her back and turned too soon.
