Absence of ovarian cysts doesn’t preclude a PCOS diagnosis. Clinically, it involves hyperandrogenism, ovulatory dysfunction, and/or polycystic morphology. Relying solely on imaging can delay detection, ignoring critical metabolic and endocrine markers. Early hormonal profiling and symptom tracking are essential for accurate identification and intervention.
