"Wrong signalling" was the main reason behind the triple train accident in Odisha's Balasore in June, a report on the findings of a high-level inquiry said. It said a similar incident occurred in May 2022 at a station in Kharagpur Division. "Had corrective measures been taken, after this incident... the accident [in Odisha] wouldn't have taken place," the report said.
