The National Disaster Management Authority in 2014 listed causes for stampedes, including structural lapses like a collapse of barricades, absence of emergency exits etc. It said fire, under deployment of security officers to control crowd, coordination gap between agencies and the decision to remove barricades also cause it. A rush of people towards entrances/exits also triggers stampedes.
